Vehicle ID Form

Use this form to complete your Vehicle Identification Information to get your sticker.

Download and complete this form and drop off at the Public Safety Office at the main gate.

Remember, we can not mail stickers, they MUST be installed here by Security staff.

We also wish to remind property owners and tenants with our vehicle ID stickers that if you sell the car or otherwise get rid of it, please remove the stickers and return them so we may keep proper track of who has these stickers. When you come to Sleepy Hollow with your new vehicle we will place new stickers on it. Because security staff must apply the stickers, we can’t mail them. These are for the new triangle stickers (not the old square stickers) and must be adhered to both Property Owner and Tenant vehicles.
